Substernal Goiter Surgery: Why This Operation Is Different and Why You Need an Expert Surgeon

A substernal goiter is not simply a large thyroid. It is a thyroid that has grown downward into the chest, and removing it safely requires a different level of surgical experience than a standard thyroidectomy.

What a Substernal Goiter Is

A substernal goiter is a thyroid gland that has enlarged to the point where a portion of it extends below the clavicles and into the upper chest cavity. Most arise from the lower poles of the thyroid lobes and descend gradually over years. In some cases the substernal component is small and incidental. In others, the gland extends deeply into the chest, compressing the trachea, esophagus, and large blood vessels that supply blood to the heart. Regardless of the underlying diagnosis, a goiter that has descended into the chest eventually requires surgery in most patients — it will continue to grow and compressive symptoms will worsen with time.

Symptoms Caused by a Substernal Goiter

⚕ Clinical note

A simple bedside test called the Pemberton maneuver can demonstrate venous obstruction from a substernal goiter. The patient raises both arms above their head and holds them there for one minute. Facial flushing, neck vein distension, or dizziness during this maneuver suggests significant vascular compression. A thorough preoperative examination should include this test.

How This Operation Differs From Standard Thyroidectomy

Substernal goiter resection is performed through the same cervical incision used for standard thyroidectomy in roughly 80 to 95% of cases. The surgeon delivers the substernal component upward through the neck incision, avoiding the need to open the chest in most patients. In a minority of cases — typically when the goiter is very large or extends into the lower mediastinum — a partial sternotomy (splitting the upper breastbone) may be required. Patients should be aware of this possibility before the operation.

Operating time is typically two to three hours, significantly longer than standard thyroidectomy. The increased time reflects the complexity of dissecting carefully around compressed and distorted structures and delivering a large gland from a confined anatomical space.

⚕ Clinical note

Unlike standard thyroid surgery, substernal goiter resection requires a CT scan of the neck and chest to map the full extent of the gland, assess the degree of tracheal and vascular compression, and identify the relationship of the substernal component to surrounding mediastinal structures. A surgeon who proceeds with this operation based on ultrasound alone does not have the information needed to plan the safest approach.

The Risks of Substernal Goiter Surgery

The risks include all of those for standard thyroidectomy, but several are meaningfully elevated due to anatomical complexity and longer operative time. Recurrent laryngeal nerve injury risk is amplified — the nerves may be stretched, displaced, or thinned by years of pressure from an enlarging gland. Intraoperative nerve monitoring is not simply recommended for this operation: it is essential.

Hypoparathyroidism risk is higher because the blood supply anatomy is often distorted by the enlarged gland and the dissection is more extensive. Bleeding risk is also elevated — large goiters have an expanded blood supply with larger and more numerous vessels, and the substernal component is adjacent to major mediastinal vessels. Patients are monitored overnight in hospital for this reason.

Long-standing tracheal compression can also cause tracheomalacia — softening of the tracheal cartilage. When the compressive force is suddenly removed after surgery, a softened trachea may partially collapse, creating airway obstruction in the post-operative period.

⚕ Clinical note

Substernal goiter resection is not a same-day operation for most patients. The combination of longer operative time, higher bleeding risk, the possibility of tracheomalacia, and the need for close calcium and airway monitoring makes an overnight hospital stay the appropriate standard of care.

Why Surgeon Selection Is the Most Important Decision

Substernal goiter surgery is not a procedure that belongs in the hands of a surgeon who performs it occasionally. The anatomical complexity, distorted landmarks, proximity to major vascular structures, and the elevated complication profile all require a surgeon who operates on substernal goiters regularly. Ask specifically how many substernal goiter resections they perform per year, whether they have experience with sternal approaches when needed, and whether they use intraoperative nerve monitoring and parathyroid fluorescence imaging routinely.

A substernal goiter is not a bigger version of a standard thyroid operation. It is a categorically more demanding procedure, and the surgeon performing it should be chosen with that fact clearly in mind.
